Age-friendly Sustainable Smart and Equitable Technologies for Aging in Place (ASSETs for AIP) demonstration program is developing and testing sensor-enhanced, team-based care coordination models for clinical translation. Vital to this work are in-home, residentially installed sensors and digital health technologies to understand client daily living patterns and functional change. Care coordinators partner with clients for healthy life style goal setting, home safety modifications, and improved communication strategies with health care teams and community support contacts.
